Privacy Policy 

This privacy policy has been written to comply with Australian Privacy Principle 2014. www.privacy.gov.au www.comlaw.gov.au 

Information gathering 

I, Tracey Morter am director of my nutritional consultancy practice. In order for me to do my work I need to gather information about your current, past and family health, it is routine that I request a thorough and detailed health history, I will not collect information that is not reasonably necessary for us to do our job. This information is gathered by you completing my client form, prior to first appointment  and verbally during your scheduled appointments. With your consent I may also request information from other healthcare providers including test results. Your information will not be shared without your prior written consent, unless I am obliged to do so by law. 

Record keeping and access to my records?

Records are kept in hard copy and/or electronically. Hard copies are kept in a lockable filing cabinet and are only accessed by Tracey Morter. Electronic records are password and authenticator protected and are only accessible by Tracey Morter. 

Sharing information electronically 

I will electronically send you your blank new client form. You can either email your completed form and results back to me or print off a hard copy and return them by post or drop them off at the clinic directly. I do not take responsibility for how you handle or share your information. 

Sharing information with oversea entities 

As a part of the privacy policy regulations we need to declare if and how information may be shared with overseas entities. We will never share your information with oversea entities without your prior written consent.

Accessing your personal information 

You may at any time request a copy of your history and or results. This request must be made in writing. We will provide you with this information in a reasonable amount of time. A report writing fee may apply in some cases, the fee will reflect the amount of time taken. No fee will ever be charged for you requesting your information. 

Minors 

A minor is a child under the age of 16. Any child under the age of 16 must be accompanied by a parent or legal guardian at all times. 

Making a complaint 

General Complaints: If you are unhappy with the service you have received or feel you have been mistreated, please send your complaint to me directly at tracey@traceymorter.com
